• enrolls every evaluator in the Proactive Disclosure Service of the National Practitioner Data Bank, and
• assures that all evaluators undergo credentials verification in compliance with nationally-recognized standards.

Physicians and allied healthcare professionals are required to submit a completed CredentialMed comprehensive Credentialing Services Packet available online or from our professional recruiters, which includes attestations on conflict of interest, confidentiality agreement with HIPAA acknowledgement, credentialing and re-credentialing forms and related materials.
